( – Full-Time Day position (7:00am – 3:00pm), flexible hours 64 – 80 hrs per two week pay period, including every other weekend).

LNA's- Mayo Rehabilitation and Continuing Care -SIGN ON BONUS PAY- Full-Time Evening shift (2pm-10pm) or Full-Time Day shift (6:00am - 2:00pm) position available

We currently have  full-time LNA positions  available on the Day and  Evening shift.

Evening Shift :  2:00pm – 10:00pm (work 72 hours per two week pay period, get paid for 80)

Day Shift :  6:00am – 2:00pm

 

Every other weekend and one or more holidays per year required. Responsible for delivering direct care to frail, elderly residents in a long term care and short term rehab not-for-profit facility. Includes every other weekend.

MDS COORDINATOR-Mayo Rehabilitation and Continuing Care- Part time

Mayo Healthcare is a small dynamic non-profit organization located in Northfield, Vermont. Mayo is currently looking for a part-time MDS coordinator. We are an award winning organization that supports a diverse workforce.

Primary Function:

  • Prepares and enters all federal and state mandated documentation & reporting requirements relative to MDS activity/tracking
  • Demonstrates a full understanding of the RAI user’s manual for Minimum Data Set (MDS) 3.0
  • Understands and accurately follows procedures for completing the care assessment area
  • Able to develop formal individualized care assessment area
  • Assesses the resident, reviews/researches the resident chart, MAR, treatment record, and LNA documentation tool, restorative aide flow sheets and PT/OT or SLP notes for information to support RAP/care planning decisions and MDS entries
  • Completes the designated sections of the MDS forms per observation of resident, information on the chart, and through interview of resident, family member and other staff. Interviews include BIMs, pain and PHQ-9 on all LTC residents. Promotes resident and family participation to establish/maintain trust and good working relationships for the residents benefit
  • Completes CAAs (Care Assessment Area) triggered from the MDS to identify potential/actual problem areas that need to be addressed in RCP to enhance resident’s care & quality of life
  • Completes any and all other assessments viewed as necessary or helpful to the assessment process
  • Enters ICD-10 Diagnosis codes and updates as needed
  • Enters MDS data into computer program per regulatory time requirements
  • Prepares list of completed MDS assessments to be submitted weekly to the state
  • Updates care plans to reflect current and accurate resident profile in a timely manner as condition warrants
  • Coordinates schedule for resident MDS assessments per regulatory requirements
  • Prior to the beginning of each month, schedules monthly nursing assessments in conjunction with MDS assessments due date

Required Skills:

  • Strong computer skills
  • Strong interpersonal, organizational, time management and written and verbal communication skills
  • Able to work independently, and complete work accurately and in a timely fashion

Required Experience:

  • Current Vermont Registered Nurse License or Licensed Practical Nurse
  • Experience in an MDS role preferred

You must be a licensed LNA to be the transport aide.

DIETARY MANAGER- Full Time - SIGN ON BONUS

Mayo healthcare located in Northfield, Vermont is currently looking for an experienced and qualified Head Chef to oversee cooking, plating, and service of all meals. This individual will be passionate about creating high quality, nutritious, thoughtfully presented meals and must possess a commitment to offering the highest standard of customer service to our residents and staff. This is a salaried position where pay is  based on years of experience. We are also offering a $3,000 sign on bonus!

Qualifications:

  • At least 5 years foodservice management experience and experience cooking high quality food from scratch in a commercial or non-commercial setting.
  • Familiarity with a wide variety of international cooking such as American, Italian, Mexican, etc.
  • Strong leadership skills.
  • Excellent organizational and communication skills.
  • Willing to be Serve Safe certified.
  • Certified Dietary Manager Preferred.

Responsibilities include:

  • Supervises the staff in the daily operation of the department, monitoring job assignments and quality of food preparations.
  • Responsible for menu development.
  • Participates in the recruitment and selection of staff and provides orientation to new staff members and ongoing coaching to current staff.
  • Prepares and submits food and supply orders, audits supply stock, and ensures that the department has sufficient food supply and working equipment at all times.
  • Communicates company policies and health department standards to all staff holding staff accountable for preparing, serving and storing food in accordance to all standards and policies.
  • Evaluates staff performance and provides feedback, coaching and discipline as needed to staff members.
  • Inspects the food service areas and equipment ensuring they are clean, safe, and orderly meeting community fire safety, infection control and hazardous chemicals regulations and policies. Taking prompt action to address any unsafe conditions or equipment problems.
  • Greets and obtains feedback regarding the dining experience from residents, visitors, and staff courteously, respecting individual dignity, resident rights and confidentiality standards at all times.
  • Keeps the dietary department within budget, maintaining records that reflect food, supply, and labor costs, and coordinates dietary services with other departments as requested.
  • Works with the dietary consultant and other department heads, reviewing community and individual resident menus and works on developing service plans.
  • Assists in planning and preparing for parties or special meals.
  • Performs other related duties as assigned.
